Taking a look at your picture, the things I can suggest are:

Give it a dark background- as it looks partly finished, I imagine the plain white background wasn't intended to be part of the fnished piece anyways, but thought I'd mention it.

Make the rims of his glasses thinner and sharper-looking, it should help them look more like we're seeing them at a 3/4 angle, as they seem a bit flat right now. Putting some shadow on his face behind them and emphasizing the dimensions of his brow with more shading should help.

His eyebrows seem a bit high- I've attached a somewhat derpy redline showing where I think they should be, according to where his eyes would be. The dark blue is a suggested alteration for the shading around his eyes.

Other than that, it looks great to me. It's a very identifiable picture of the Medic, with a diabolical expression. And to answer your other question... usually, I just daydream or toss ideas back and forth with my sister.
